Making an Appointment
To make an appointment, please call our office at 404-508-2000. Alternately, you may submit the form below. You will receive a phone call from one of our schedulers to verify your information.
Before calling:
- Please have your insurance card available.
- Please obtain a referral if your insurance plan requires one.
Preparing for the appointment:
- Please download the new patient packet, complete the forms, and bring them on the day of your appointment.
- Please bring your insurance card and a photo i.d.
- Please bring your co-payment and/or other funds required to pay for your visit. Payment is due at the time services are rendered.
- Accepted methods of payment: Cash, Credit Card, Personal Checks
- The hospital no longer allows parking validation. Please be prepared to pay for parking.
